Frank A. Agnone, M.D.
Frank A. Agnone, M.D. is a board certified Internal Medicine Physician who has been practicing medicine in the Phoenix Valley since 1988. His training includes a Medical Toxicology Fellowship, a General Surgery Internship and a Medical Residency. He believes that the power to transform a patient’s life is lurking in every clinical encounter. Solid clinical practices applied to a patient’s desire to embrace the terms of recovery are what generate good clinical outcomes and the very restoration of health. He resides in Phoenix with his wife and three children.
Ann Lacey, F.N.P
Ann is a board-certified Family Nurse Practitioner. She earned a Bachelor’s Degree in Nursing from Arizona State University in 2000 and a Master’s Degree in Nursing from the University of Phoenix in 2011. She is a member of the American Academy of Nurse Practitioners and stays actively involved with the American Nurses Association as well as the Arizona Nurse’s Association. Her background includes over a decade of Nursing Practice in the Critical Care setting with the acutely-ill. She has extensive experience as a Certified Critical Care Nurse involving Cardiovascular Nursing.
Ann believes four modifiable health risk behaviors (lack of physical exercise, poor nutrition, tobacco use, and excessive alcohol use) are largely responsible for much illness, suffering, pre-mature aging, and chronic disease. Therefore, she is passionate about connecting with patients about innovative approaches to empower them to attain their best level of wellness through preventative medicine and healthy lifestyle choices.
She is originally from Phoenix, Arizona and enjoys all that Arizona-living has to offer with her husband of 23 years and their four children.
Shanna Carter, FNP-C.
Shanna began her nursing career as a licensed practical nurse working with the HIV population. After five years she then went on to pursue her RN-BSN, working in a wide array of nursing fields, including ventilator management, neuro-trauma step down, wound management long term care, and home health/hospice. After 15+ years in nursing, she then obtained her family nurse practitioner master’s degree from Northern Arizona University, with honors. She has specialty knowledge on chronic disease management with an emphasis in adult preventative health.
Shanna is married with 2 adult children and lives in Queen Creek. In her free time she enjoys reading, traveling, exercise/Zumba, and spending quality time with family and friends.
Angela Acquafredda, MSN, FNP-C.
Angela Acquafredda, MSN, FNP-C is a Board Certified Nurse Practitioner and has been in practice for the past 2 years in Internal Medicine. She is also the Director of an Ambulatory Surgery Center in Phoenix.
Angela attended Grand Canyon University where she obtained her Master’s degree with Honors in 2016. She had been a RN prior since 2008 in various specialties and leadership roles, including obstetrics/gynecology, labor and delivery, public health, urology, and gastroenterology/endoscopy.
Angela is an active member of the American Academy of Aesthetic Medicine (AAAM), American Association of Nurse Practitioners (AANP), American Diabetes Association (ADA), the Association for Professionals in Infection Control and Epidemiology (APIC), and the Society of Gastroenterology Nurses and Associates (SGNA).
Christine Minch, DNP, APRN, FNP-BC
Christine Minch is a doctorally-prepared and board-certified Family Nurse Practitioner. Her formal education includes a Bachelor of Science in Nursing from Arizona State University in 2001, a Master of Science in Nursing in 2014, and as her profession’s highest degree, a Doctor of Nursing Practice from Grand Canyon University.
In addition to the extensive training Christine received from Dr. Frank Agnone, her professional background includes considerable experience in healthcare leadership and management, along with nursing specialties in critical care, geriatrics, and palliative care—affording her the clinical acumen to render comprehensive primary care to patients as an advanced practice nurse. Christine promotes the integration of evidence-based practice and patient empowerment to achieve optimal health outcomes.
Christine is an invited member of Sigma Theta Tau International Honor Society of Nursing, as well as a participating member of the American Association of Nurse Practitioners. She is fluent in Tagalog and conversant in Spanish. In her leisure time, Christine enjoys cultivating a plethora of hobbies and is an avid traveler with her husband.
